Chinese Medical Reality Show Draws Scrutiny Over Contestants’ Clinical Skills

A reality show following medical students has triggered debate in China after its premiere highlighted gaps in clinical skills, training and medical judgment among several contestants.

The eighth season of the Chinese reality show “An Exciting Offer” has come under scrutiny after its first episode showed medical students guessing at diagnoses, struggling with surgical knots and making remarks that viewers questioned on ethical and professional grounds. The Tencent Video program premiered on Oct. 21.

This season brings together 11 contestants, ranging from undergraduates to doctoral students in several specialties. They compete for an internship at Nanfang Hospital of Southern Medical University in Guangzhou, the capital of Guangdong province. The program uses scenarios modeled on workplace situations. Earlier seasons focused on aspiring lawyers, television presenters and architects; the first season remains its highest-rated on Douban, with 8.2 out of 10.

Much of the criticism focused on Li Ziqi, a neurosurgery doctoral student at Fujian Medical University. Li said he had taken part in Level 4 surgery, the highest category in China, but answered a question about his surgical ability by saying that doctoral students did not perform clinical work during their studies.

Medical students watching the show disputed that statement, noting that candidates pursuing professional doctoral degrees normally receive clinical training. Online users also examined Li’s academic record and said his papers were not highly ranked under the Science Citation Index Expanded classification system.

Fujian Medical University later told domestic media that Li had completed 18 months of clinical practice. The institution said his claim that doctoral students did not undertake clinical work was inaccurate and had caused a misunderstanding.

Other scenes intensified the debate. Li told fellow contestants that medical-record writing could be learned in a day. During a diagnostic exercise, he said he was “guessing” that a case involved a craniopharyngioma, a type of brain tumor, without providing supporting evidence. A mentor with experience in several medical fields told him that a diagnosis could not rest on guesswork.

Viewers said those exchanges appeared to minimize the importance of clinical training. Another contestant, reported to be an intern at a prominent private hospital in Sydney, needed instruction on tying surgical knots. The only undergraduate participant said she would personally pay for cosmetic suturing for a patient, prompting questions about her understanding of medical ethics.

A highly rated Weibo comment from a user identifying as a medical student said standardized residency training involved continuous case reports and consecutive shifts, while the program made clinical work appear much easier. Other viewers worried that an emphasis on mistakes could deepen public distrust of doctors.

The debate also exposed confusion over China’s two postgraduate medical pathways. Academic degrees emphasize research, while professional degrees prepare students for clinical practice. Critics said the program did not clearly explain that distinction, leaving audiences without important context for judging the contestants’ experience.

A commentary in the state-run People’s Daily argued that a program built around doctors had turned diagnosis, professional development and surgical boundaries—areas where mistakes carry serious consequences—into material for performance.

The contestant reported to be working in Sydney responded on Xiaohongshu, also known as RedNote, saying the experience had made her recognize differences between Chinese and Western medical training. She acknowledged that she still needed to improve her basic skills and practical experience. The production team had not issued a response to the controversy at the time of publication.
